Good Morning to one and all

  I am one of the patients of your prestigious Medical Center last Tuesday, January 27, who had undergone Magnetic Resonance Imaging (MRI). I wish to thank all the staff/employees who extended their unselfish and dedicated assistance while I was at the Center.  Special mention to Dr. Hidegunda Santos, Medicard coordinator, the HMO staff (the two ladies from the billing section), Mr. Oliver Hufana and his staff (MRI section), and the nursing students who assisted at the MRI section.

“A smiling face is the mirror of our body and can heal sick patients” – Last March I came here for check-up. I was then referred to Dr. Espinosa so I went to his clinic. He sent me back here for EEG. The staff downstairs at the front office did the EEG for me right away so I had the time to go back to Dr. Espinosa who interpreted the result. It was done so fast that we had the time to go back to Ilocos that same day. I thank the staff very much. To Lorma as a whole, thank you so much and may God bless you all. All the staff and employees of Lorma Hospital are very friendly. I’m very glad to see their smiling faces every morning. Keep up your good work!!!

The following describes benefits for LMC personnel and their families.

HOSPITALIZATION

For the further enhancement of the ultimate welfare of employees, the following additional benefits and privileges are provided:

  1. 100% discount on professional fee for dependents
  2. Discount to be applied after Medicare for:

  1. FREE hospitalization expenses for surgery cases net of Medicare, Insurance and other benefits.
        -If the employee chooses a doctor other than Lorma staff, he/she should pay the I bill of that doctor .
        -If the operation is really needed and the hospital staff cannot do the operation, then the hospital will pay the fee of the other surgeon. -MEDICO-LEGAL CASES: NOT COVERED.
  2. FREE hospitalization expenses for DELIVERY for the first TWO live born babies of a regular female employee.
        -If the employee had her first baby before she was employed, only her second baby will be free.
        -Baby's bill whether Caesarean or normal delivery is not free but subject to the usual dependent's discount.
      

                NOTE: If the employee chooses a room higher than the allowed per category he/she should
                pay the excess in room and other expenses. b. Take- home medicines are not free.
 

DEFINITIONS OF DEPENDENTS:

FOR MARRIED EMPLOYEES -the legitimate spouse or legitimated children who are
under 21 years of age, not gainfully employed; children who are physically incapacitated; and the parents of the employee who are 60 years old and above, wholly dependent upon him for regular support and reside with the employee.

FOR UNMARRIED EMPLOYEES -the tent1 aged parents means those parents who are actually living with the employee and are completely dependent upon the latter for regular support and sustenance.

EDUCATIONAL BENEFITS (AT LORMA COLLEGE)

1. Employees of 1-2 years of service
    (a) Ist child 10%
    (b) 2nd child 20%
    (c) 3rd child 30%
2. Employees with 3-5 years of service
    (a) Ist child 15%
    (b) 2nd child 35%
    (c) 3rd child 55%
3. Employees with more than 5 years of service
    (a) Ist child 25%
    (b) 2nd child 50%
    (c) 3rd child 75%
4. Employees with more than 10 years of service   
    (a) Ist child 50%
    (b) 2nd child 75%
    (c) 3rd child 100%

EMPLOYEE HOSPITALIZATION, OPD AND DRUG CHARGES

  1. The maximum credit granted to each employee is 3 times his basic monthly salary or P5, 000 whichever is higher .
  2. If the charges exceed the above amount, only the employee's hospitalization will be granted credit and the accounts for relatives and friends cannot be added to the employee's account.
  3. The monthly deductions will be based on the following font1ula: The amount outstanding should be divided by 10 to give the proper amount. However, in no case shall this be more than 15% of the employee's gross monthly salary, or less than P20, 000. 4. The deduction is made at the .15th and at the end of every month.

ANNUAL PHYSICAL CHECK-UP

An employee shall be given FREE routine medical check-up every year to include physical examination, chest x-ray, routine blood, urine, stool examinations and ECG for employees above 40 years or upon request of attending physician.

All permanent employees who are directly exposed to patients are given FREE testing and vaccination for Hepatitis B provided found to be non-reactive (negative).

CT SCAN DISCOUNT for STOCKHOLDERS and EMPLOYEES

A. Stockholders -50% discount of the P3,000 or Pl,500
from Dr. Jerome (This refers to the: ISIC stockholders LORMA, Inc. Board ofDirectors President of Lorma Cooperative)
B. Spouse/Children -25% of the P3,500
C. Brothers, sisters, parents -10% of the P3,500
D. Employees, Lorma Corp. -25% of the P3,500 and ISIC -50% of the P3,500
E. Doctors -10% of the P3,500
F. HOSPITAL ACCOMMODATIONS FOR EMPLOYEES:
DIVISION CHIEFS Any suite/Executive suite except Ambassador, Presidential and Macagba suites
DOCTORS/DEPARTMENT HEADS Aircon Private Rooms (Station A only)
NURSES and other Professionals Private Rooms
Other Employees Semi-Private Rooms

Note: An employee may choose a higher room but the excess charge from Medicare should be accounted to him.
