“A smiling face is the mirror of our body and can heal sick patients” – Last March I came here for check-up. I was then referred to Dr. Espinosa so I went to his clinic. He sent me back here for EEG. The staff downstairs at the front office did the EEG for me right away so I had the time to go back to Dr. Espinosa who interpreted the result. It was done so fast that we had the time to go back to Ilocos that same day. I thank the staff very much. To Lorma as a whole, thank you so much and may God bless you all. PATIENT'S BILL OF RIGHTS

Realizing that cure without care strips persons of their dignity we strive to maintain personalized service amid our technical progress and expansion. Thus, Lorma Medical Center holds to the ideals of Christian concern, justice and truth believing that ultimately our mission of care is to heal the whole man. As patient at Lorma Medical Center , you can be assured of the following:

  1. Considerate and respectful care.
  2. Upon request, the name of the physician responsible for coordinating your care.
  3. The name and function of any person providing health care services to you.
  4. Obtaining from your physician complete current information concerning your diagnosis and prognosis in terms you can reasonably expect to understand. When it is not medically advisable to give such information to you, the information shall be made available to an appropriate person in your behalf.
  5. Receiving from your physician information necessary to give informed consent prior to the start of any procedure or treatment or both and which, except for an emergency situation not requiring an informed consent, shall include as a minimum the specific procedure or treatment or both, the medically significant risks involved, and the probable duration of incapacitation, if any.
  6. Refusal of treatment to the extent permitted by law and to be informed of the medical consequences of your action.
  7. Privacy to the extent consistent with providing adequate medical care to you. This shall not preclude discreet discussion of your case or examination of you by appropriate health care personnel.
  8. Privacy and confidentiality of all records pertaining to your treatment, except as otherwise provided by law or third party payment contract.
  9. A response by the hospital, fu a reasonable manner, to your request for services customarily rendered by the hospital consistent with your treatment. .
  10. Information by your physician or his delegate as to your continuing health care requirements following discharge and that before transferring you to another facility the hospital first informs you of the need for such a transfer.
  11. The identity, upon request, of other hospital care and educational institutions that the hospital has authorized to participate in your treatment.
  12. Upon request, an explanation of your bill, regardless of source of payment.
  13. Being advised of the hospital rules and regulations as they apply to your conduct as a patient.
  14. Treatment without consideration as to race, color, religion, sex, national origin or source of payment.

If we fail in any way to meet your needs, or if you have questions which remain unanswered, please contact your Patient Representative...an advocate trained to act in your behalf. Or, call for the MEDICAL DIRECTOR, ADMINISTRATOR, or CHIEF NURSE. They may be reached through the information/admitting section.
