Good Morning to one and all

  I am one of the patients of your prestigious Medical Center last Tuesday, January 27, who had undergone Magnetic Resonance Imaging (MRI). I wish to thank all the staff/employees who extended their unselfish and dedicated assistance while I was at the Center.  Special mention to Dr. Hidegunda Santos, Medicard coordinator, the HMO staff (the two ladies from the billing section), Mr. Oliver Hufana and his staff (MRI section), and the nursing students who assisted at the MRI section.

“A smiling face is the mirror of our body and can heal sick patients” – Last March I came here for check-up. I was then referred to Dr. Espinosa so I went to his clinic. He sent me back here for EEG. The staff downstairs at the front office did the EEG for me right away so I had the time to go back to Dr. Espinosa who interpreted the result. It was done so fast that we had the time to go back to Ilocos that same day. I thank the staff very much. To Lorma as a whole, thank you so much and may God bless you all. All the staff and employees of Lorma Hospital are very friendly. I’m very glad to see their smiling faces every morning. Keep up your good work!!!

US VETERANS, who are TRICARE members,

now covered for medical services at Lorma Medical Center,

San Fernando City, La Union, Philippines

 

The uniformed services covered include US army, navy, air force, marine corps, coast guard, US public health services, national oceanic & atmospheric administration.
 

FOR DEPENDENTS & NON SERVICE-RELATED CONDITIONS:


TRICARE standard is a cost sharing program for US vet retirees & families. Beneficiaries pay a deductible. To be eligible, they need to be a TRICARE member.

To enroll a dependent with TRICARE, or to verify eligibility, contact any of the following:
  1) DEERS - Defense Enrollment Eligibility Reporting System at 1-800-538-9552

All the beneficiary has to do is present his ID at Lorma Medical Center. Lorma will verify his status. Lorma Billing Department will file the claim. As long as beneficiaries are paying their deductible, they are eligible & Lorma can verify their status online. For those who come to Lorma for the first time, they register by filling up a form.

 FOR SERVICE-RELATED CONDITIONS:

US Veterans should have prior authorization from the Veterans Administration in PAsay City and covered by a Letter of Authorization (LOA).

1. OUT-PATIENT CARE (consultation and diagnostic/therapeutic services)

Procedure:

A) LOA (VA Form 10-7079) presented by USVA Veteran to Lorma Corporate Services Health Coordinator

 

B) Corporate Services Health Coordinator escorts patient to doctor of choice and to other pertinent departments

 

C) When visit is over, Corporate Services Health Coordinator advises patient that USVA will cover outpatient care

 

2. IN-PATIENT CARE (bed/board, diagnostic/therapeutic services, drugs & pharma supplies)

Procedure:

A) USVA Vet goes to Lorma ER and is admitted

 

B) US Vet is given 72 hours to contact USVA, Manila to obtain LOA (VA Form 10-7078) or

Lorma, through its Billing staff, can do this in behalf of the patient.

 

C) USVA faxes LOA to Lorma Billing

 

D) Upon discharge, Billing Staff advises patient that Lorma will process claim in behalf of patient
